Responsibilities: The Patient Care Coordinator Licensed Practical Nurse under the supervision of the Oncology Nurse Manager, the Patient Care Coordinator LPN coordinates the referral process for medical and radiation oncology patients, including second opinions at centers across the country, stem cell and bone marrow transplant referrals, home care services, hospice, and diagnostic testing.
The Patient Care Coordinator arranges home chemotherapy infusions, facilitates the delivery of controlled substances for hospice patients, and obtains access to antineoplastic drugs from specialty pharmacies and pharmaceutical companies.
The Patient Care Coordinator bridges coverage gaps for patients to ensure timely treatment and maximize financial benefit for patients.
The Patient Care Coordinator is responsible for accurate, timely, and thorough review of referrals, patient advocacy for access to care, and appropriate scheduling and tracking of appointments for patients referred by BCI providers to non-Bassett specialists, clinics and treatment centers.
The Patient Care Coordinator provides clinical information to case managers at insurance companies on behalf of the BCI patients to obtain medically necessary treatment and services.
The Patient Care Coordinator I also serves as an effective liaison between BCI providers, non-Bassett specialists and clinics, patients, and managed care administrative staff.
Provides care and coordinates treatment for Cancer Institute patients.
Ensures patient referrals within the Bassett Network and to outside medical service facilities are completed in a timely manner as observed by direct supervisory observation and chart documentation.
Obtains prior authorization for diagnostic tests, treatment plans, and medications to ensure adherence to the prescribed plan of care.
Documentation of authorization is completed in the patient record.
Coordinates on time delivery of cancer drugs to patients through home infusion services, specialty pharmacies, and local pharmacies as evidenced by random supervisory observation and documentation in the patient record.
Completes patient referrals and facilitates the intake process for hospice and palliative care agencies including verification of medical condition, medication lists, and prescriptions.
Investigates financial resources for patient assistance including pharmaceutical sponsorship, not-for-profit funding, and patient assistance programs and enlists the assistance of social work as necessary.
Provides coverage for the critical value lab pager and prescription phone line.
Advocates for patients to ensure that care preserves and protects the patient’s autonomy, dignity and rights.
Delivers care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversity as measured by no significant, substantiated patient or peer complaints received by the manager.
Assures that initial care processes are followed for patients who are prescribed oral chemotherapy regimens, including patient education by RN, prescriptions, and consent.
Maintains current licensure and competencies.
Provides to supervisor evidence of completion of all required annual training and competency validation.
Provides supervisor with evidence of completion of continuing education credits related to oncology patient care.
Four hours are required to meet the standard.
Practices within the New York State-defined LPN scope of practice.
Performs similar or related duties as requested or directed Qualifications: Education: Graduation from a Practical Nursing program, required Experience: Minimum one year experience in oncology nursing, preferred Experience with obtaining referrals and accessing patient services, preferred License/ Certification: Current NYS Nursing License as an Licensed Practical Nurse, required Registered Nurse license would be accepted BLS certification must be obtained within 6 months days of start date, required Skills: Attention to detail Build Relationships Clinical Skills Communication Computer Skills Multi-Tasking Reading/Writing/Comprehension Time Management Physical Requirements: Medium Work - Exerting up to 50 lbs.
