*SUMMARY*
Responsible for the development and monitoring of program plans and individual budgets in the Self Direction Program.
Provides initial staff training on the individual specific plan to the staff responsible for implementation of the plan to the individual.
This position includes providing Agency Start Up/Support Broker services as requested.
Brokers assist individuals to self-direct and co-manage the services and supports they receive.
The Broker is responsible to create, update and review with the person their individual budgets annually and more often as needed.
The self-directed services Broker helps the person who wants more control and choice in the supports and services they receive to help them achieve their personal outcomes.
This position will also be responsible for overall delivery of services as needed.
*ESSENTIAL DUTIES AND RESPONSIBILITIES* *Program Coordination* * Implement assessment recommendations, develop plans to support individual’s outcome pursuits.
* Facilitate the development and implementation of each person’s program plan assigned to caseload.
* Write skill acquisition and support programs with the person and with input from other professional staff and those assigned direct responsibility for implementation of the program plan.
* Monitor program plans to ensure accurate and pertinent value to each individual’s personal goals.
* Provide accurate and consistent communication with all members of the individual’s team, including parents, guardians, professionals, and those responsible for implementation of the program plan.
* Complete and maintain current agency training requirements pertinent to job duties.
* Train, coach, and monitor staff for implementation of the program plan.
* Implementation of direct support services may be required.
*Record Keeping/Financial Responsibility* * Ensure that all service provisions meet regulatory requirements, ensure documentation is complete in a timely and legible manner.
Follow-up with staff to ensure billing is accurate and correct.
* Maintain meeting minutes and documentation of team contacts.
* Ensure that each individual receiving services has an accurate and complete record.
* Complete quarterly QA checks on each individual’s records.
* Maintain accurate accounts of program enrollment, unit utilization and budget line items.
* Create and submit all programs, forms and reports as due.
* Ensure monthly billing is accurate for each person and submitted in a timely fashion.
* Complete all other reports as needed and requested by the department/agency.
* Keep expenses within the program budget allocations.
*Plan Writing* * Complete individual assessments prior to the creation of the plan and update as needed.
* Develop and implement each person’s program plan as directed by that person, and per the Office for People with Developmental Disabilities (OPWDD) ADM regulations and guidance.
* Monitor the individual’s progress on a monthly basis with interventions as necessary.
* Complete and submit monthly summary on each training program with updates as necessary.
* May assist with plan writing for other community programs such as SEMP, Community Pre-Vocational Services, or Pathway to Employment as time allows.
*Broker Services* * As startup broker, assist in design and completion of an approvable SD Plan/Budget.
* Support Broker- provide a variety of long-term supports and services to ensure successful implementation and continuation of the approved Plan/Budget.
The primary function of the support broker is to maintain the SD Plan/Budget.
The broker will facilitate the person-centered planning process, help the person develop a circle of support, provide training and support to help them gain the skills they need to hire and supervise staff, document services, etc.
* Support Brokers must have at least two face-to-face Circle of Support/Planning Team meetings with the individual during a one-year period.
This one-year period begins on the date that the Broker signs the Support Broker Signature line on the initial Support Broker Agreement between the individual and the Support Broker.
The one-year period restarts on the anniversary date of the Support Broker Signature.
* Monitor budget expenditures and make necessary and requested budget updates to ensure continued successful and complete service provision.
*Other* * Other duties as assigned * This position requires a three-week (21 days) notice for voluntary resignation of employment.

Additionally, 12 hours per year of professional development is required to maintain broker status.

Ability to write reports, business correspondence, and procedure manuals.

Ability to apply concepts of basic algebra and geometry.

Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
